You and your child may need extra immunisations when going abroad.  Before you travel we would suggest you make an appointment with our Practice Nurse for some advice.

To ensure that you are given the most up to date advice, the Practice Nurse will need some information regarding your travel details.  Please make an appointment with the Practice Nurse at least 6 weeks before your travel date if possible.

Please contact the reception team to book a routine appointment with the nurse for your initial consultation.  At this appointment your immunisation history will be reviewed and any immunisations that you require will be discussed.

Prior to your appointment please print off the attached form and fill out, bringing the completed form with you to your appointment.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
